- Public Disclosure of Services and Limitations
The HHA will make available to the public clear and accurate information regarding the types of services rendered, including any limitations related to specialty services, service duration, or service frequency. This information will be reviewed annually or more frequently as needed if services change.
- Service Availability: Premiere Home Health provides skilled nursing, therapy, social work, and aide services.
- Service Limitations: Premiere Home Health does not provide services to patients younger than 18, and we do not provide services for patients who have IVs or are ventilator dependent. We do not provide service for patients who require twice a day visits when there is no caregiver available to provide care. Our geographic service area is a 50 mile radius from our Agency Office 926 Hemsath Road, Suite 104A, St Charles MO 63303, and is subject to current caseload and our ability to safely provide our boutique experience to the patient.
